Most insurance's will cover this procedure but you have to meet the criteria. Your doctor will have to contact your insurance company and give your measurements, weight and sometimes a picture.
You will need evidence of visits to your doctor complaining of chronic back and shoulder pain.
Your insurance company will always want you to lose some weight, like 10 to 20 pounds depending on your size.
They wil not pay for a lift for cosmetic reasons but reductions are lifts.

What do you mean "what doctor's offices get reimbursed for certain drugs" ?

If you're talking about prescriptions, the insurance company doesn't reimburse the doctor – the payment goes to the pharmacy plan.

Or are you talking about specialty injectibles administered in the doctor's office? Those are pretty much the only sort of drugs that a doctor is going to be reimbursed for by the insurance company.

If you clarify your question, you may get better responses.

I'm not aware of websites that compile that info for different insurance companies. Especially since different doctors could be contracted for different rates with any one insurer. (Depends on how good their contract negotiator is.)

The fee schedules for Medicare and Medicaid are publicly available – if you know the "J code" for the injectible, Medicare and Medicaid would give you a good starting point to get a ballpark estimate of how much to expect. (Keeping in mind that Medicaid is generally the lowest reimbursement around and any private insurer is going to pay a substantially higher rate than Medicaid.)

You could obviously find out the information from your own insurer, by looking at your Explanation of Benefits. However, I get the impression that you're looking to "comparison shop" and see what the average rate would be for other insurers. That information might be a little tough to compile.

Many contracts for pharmaceuticals are based on a markup from AWP (average wholesale price). For example, one doctor could have a contract for AWP+10% for specialty drugs. (Meaning that if AWP for your drug was $100, your doctor would get reimbursed $110.) Again, different doctor's may have different deals…one doctor in your area could be AWP+10%, while another could be AWP+12%, etc.

If you can get ahold of the AWP (average wholesale price) for your injectible, that could also help you come up with a ballpark range of what the reimbursement might be.

(Yes – I know. It all sounds confusing. I've gotten used to it because I work with this information every day. If you need any more explanation of what I've said here, I can try to come closer to putting it in plain English. lol)

She's going to have to go to a local high risk agent, and get quotes through them. Hopefully, she's not overweight, also – it's likely going to be very expensive if she's NOT overweight, and not available if she is obese (which is the case for most young type 2 diabetes people.)

She can talk to her own agent to get a referral to a high risk life guy.

It's likely going to cost 5 – 10 times as much as it would if she was healthy.

I don't think they do in general it is elective unless it is for the life of the mother. Most don't cover pregnancy either unless it is a group plan and many don't cover birth control pills and I have never heard of one who covers condoms.

I didn't have coverage at all and abortion was illegal in 1967 but when I was in the hospital they decided to do what they called a threatened abortion because they didn't know if I had already lost the baby or not. They did a D&C to stop the bleeding and may have aborted my less than 1 month old fetus but I had already had two transfusions from lost blood before they did that.

Some abortions are needed to prevent death of the pregnant woman, insurance should cover it. Rather mean spirited of you to want to have them not be covered when needed not elective.

If your breed is listed on the 'bad' list, shouldn't the insurance company look at the 'dog' itself instead of the breed? Why should I pay a higher premium if my dog has Obedience, Show titles etc…Should the insurance companies at least LOOK at the titles that the dogs have? The AKC doesn't just 'hand' out titles to dogs. They have to EARN them thru hard work between the owner and dog.

It is a shame, but it seems to be a case of the "good" owners having to pay because of the "bad" owners. I can understand the insurance company's refusal to consider AKC titles because it's not the AKC that will have to pay a claim if a situation arises…
